What Is Form 8955-SSA?

Form 8955-SSA, officially titled the Annual Registration Statement Identifying Separated Participants With Deferred Vested Benefits, is a stand-alone annual IRS form filed to satisfy reporting requirements under IRC section 6057(a). It identifies former plan participants who separated from service but still hold unpaid deferred vested retirement benefits.

The IRS forwards this data to the Social Security Administration (SSA), which uses it to identify and notify eligible individuals about their retirement benefits when they apply for Social Security.

Who Must File Form 8955-SSA?

If you are the plan administrator of a retirement plan subject to the vesting standards of ERISA section 203, you must file Form 8955-SSA. This requirement covers defined benefit pension plans, 401(k) plans, profit-sharing plans, and 403(b) plans under ERISA. Sponsors and administrators of government plans, church plans, and owner/partner-only plans are not required to file but may choose to do so voluntarily.

What Must Be Reported on Form 8955-SSA?

Form 8955-SSA covers all reportable activity related to separated participants with deferred vested benefits. The form must include:

  • The participant's full SSN, legal name, and plan sponsor information
  • The type, payment frequency, and amount of the deferred vested benefit
  • Corrections or updates to any previously reported participant information
  • Status changes, including benefit payouts and plan-to-plan transfers

What's New in Form 8955-SSA for the 2026 Tax Year?

  • Mandatory eFiling applies when a filer must submit 10 or more information returns in aggregate during the calendar year (effective for returns required to be filed on or after January 1 of that tax year),
  • Form 5558, the extension form used to request more time to file, can now be filed electronically through the Department of Labour's EFAST2 system (effective January 1, 2025).

Frequently asked questions

Form 8955-SSA has no minimum filing threshold. Filing is based on participant status. If a separated participant holds an unpaid deferred vested benefit, they generally need to be reported, regardless of the benefit amount.

A common filing issue with Form 8955-SSA is omitting a separated participant who still holds a deferred vested benefit, or not updating previously reported participant information when their status changes. Both can trigger IRS penalties.

The IRS imposes a $10 per participant per day penalty for failure to file or for omitting required participants, up to $50,000. Failing to report plan status changes carries a separate $10 per day penalty, up to $10,000.

File an amended Form 8955-SSA by checking the "amended registration statement" box in Part I. The amended filing must be complete, meaning the entire form needs to be resubmitted, not just the section with the error. Use Entry Code B in Part III for corrected participant information.
