What Is Form 5498-SA?

Form 5498-SA, officially titled "HSA, Archer MSA, or Medicare Advantage MSA Information," is an IRS information return filed by account trustees and custodians. It covers three account types:

  • Health Savings Account (HSA)
  • Archer Medical Savings Account (Archer MSA)
  • Medicare Advantage MSA (MA MSA)

The form records total contributions made during the year along with the account's year-end fair market value, giving both the IRS and account holders a verified record of annual contribution activity.

Who Must File Form 5498-SA?

Form 5498-SA is filed by the trustee or custodian of the financial institution that holds and manages the HSA, Archer MSA, or MA MSA, not by the account holder. Trustees and custodians must file for every participant whose account they managed during the calendar year.

If the same participant holds more than 1 type of account, a separate Form 5498-SA is required for each.

What Must Be Reported on Form 5498-SA?

Form 5498-SA captures the following items across its current 6-box structure:

  • Box 1: Archer MSA contributions by the employee or self-employed person for the calendar year, including amounts through April 15 of the following year (Archer MSA only; not for HSA)
  • Box 2: Total HSA or Archer MSA contributions for the calendar year, including prior-year designations and qualified HSA funding distributions from an IRA
  • Box 3: HSA or Archer MSA contributions made in the following year for the calendar year
  • Box 4: Rollover contributions received during the calendar year
  • Box 5: Fair Market Value (FMV) of the account on December 31
  • Box 6: Checkbox identifying the account type (HSA, Archer MSA, or MA MSA)

Trustee-to-trustee transfers between HSAs, between Archer MSAs, and from an Archer MSA to an HSA are not reported as contributions or rollovers.

What's New in Form 5498-SA for the 2026 Tax Year?

The current instructions for Form 5498-SA note two updates:

  1. Trustee and participant address fields are now in separate entry boxes on the form.
  2. Filers should use the December 2026 revision of Form 5498-SA and its corresponding instructions to file 2026 information. These revisions remain in effect until the IRS issues a superseding version.

Form 5498-SA Filing Requirements

Filing Form 5498-SA involves more than reporting contribution information to the IRS. Trustees and custodians must also meet several compliance and recordkeeping requirements.

To ensure accurate reporting and IRS compliance, keep the following requirements in mind:

  • eFiling is required when the total number of information returns filed in aggregate reaches 10 or more during one tax year, regardless of form type.
  • A participant statement (Copy B) must be furnished to each account holder in addition to the IRS filing. Submitting to the IRS alone does not satisfy the requirement.
  • Contributions made between January 1 and April 15 of the following year may be designated for the prior tax year. Before reporting these, collect the participant's written confirmation of which year the contribution applies to.
  • Keep supporting records on file, including contribution records, participant identification records, and account value documentation, to substantiate the amounts reported on Form 5498-SA.

Form 5498-SA Due Dates for the 2026 Tax Year

Form 5498-SA carries a single unified deadline. Both the participant statement and the IRS filing are due by May 31 of the year following the tax year. If any filing due date falls on a Saturday, Sunday, or legal holiday, the deadline shifts to the next business day.
